New Game Plus or NG+ occurs after you A: beat endless and destroy the source of immortality without beating 5 heroic bosses, B: Beat Endless and Thalamus, or C: Beat Endless and Death. Once one of these conditions are met you can move on to NG+. NG+ gets progressively harder as you go up, up until +20.
